Must-Have: 1 year Certified Home Care experience, 1 year Case Management experience
Description:
All applicants must have 1 year of Certified Home Care experience.
1 year of Case Management experience is required.
Ability to travel to cases as assigned is necessary.
Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience.
Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement.
Weekend rotations are once every 6 weeks.
As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
Patient care is delivered within organizational philosophy, policy, and standards of community health practice.
The role also requires performing admission processes and case management responsibilities in accordance with organizational policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
