Job Title: Home Health Registered Nurse Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 1 year of Skilled Home Health Experience, Oasis experience required License: Active RN license required Certifications: BCLS required Must-Have: - Oasis experience required - Reliable transportation Description: The role involves providing home health care to patients in the community. The nurse will work 5 days a week on an 8-hour shift. The patient-to-nurse ratio is 1:1, with the potential for visiting up to 6 patients per day. Experience with Oasis is mandatory. Occasional weekend work may be required. The facility follows strict COVID-19 vaccination guidelines, requiring the primary series vaccination and one booster. Candidates should have Epic experience, although it is preferred, not required. The successful candidate will be skilled in adult home health practices including admission assessments, medication management, and wound care. This position requires knowledge in starting and maintaining IVs and administering medications. Nurses will follow infection control precautions and conduct pain assessments using various scales. The role supports an interdisciplinary team approach to patient care. Candidates should be familiar with charting and documentation practices.
