Travel Nurse RN - Home Health - $2,724 per week in Newton Center, MA

Job Title: Home Health RN Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum of 1 year Certified Home Care experience and 1 year Case Management experience License: RN License required Certifications: Relevant certifications preferred Must-Have: - 1 year Certified Home Care experience - 1 year Case Management experience - Ability to travel to assigned cases Description: All applicants must have 1 year of certified home care experience. Applicants must possess 1 year of case management experience. Experience in hospice, private care, or working as a case manager independently does not meet the qualifications for certified home health care management experience. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Weekend rotations occur once every 6 weeks. As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within the organizational philosophy, policy, and standards of community health practice. The RN will be required to perform admission processes and case management responsibilities in accordance with relevant policies and standards of practice. Thorough and timely electronic documentation and interdisciplinary communications are essential.
