Job Summary:
Home Healthcare Nurse, Home Healthcare, Location unspecified. This full-time position involves performing comprehensive nursing assessments upon patient admission, integrating physiological, psychological, and environmental data to ensure safe and high-quality care. The role includes developing, implementing, and revising patient care plans, coordinating discharges, and documenting care activities in accordance with healthcare standards.

Job Details:

• Certifications, Education, and Experience: Current Registered Nurse (RN) license in Massachusetts and related credentials to meet specialty requirements. Minimum of one (1) year of nursing experience required, or six (6) months of nursing experience with a Bachelor of Science in Nursing (BSN). CPR certification required. Home care and intravenous (IV) experience, computer skills, and BSN preferred.
• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Responsibilities: Performs a comprehensive assessment on patient admission, addresses health history and environmental factors, and develops individualized care plans. Utilizes health care resources efficiently, participates in interdisciplinary case conferences, supervises Home Health Aide activities biweekly, and maintains accurate documentation. Notifies healthcare providers of significant changes in patient condition, completes admission and discharge paperwork, and coordinates patient discharge planning.
• Submission Requirements: Candidates must be licensed in Massachusetts and meet specialty and experience qualifications.
