You will implement care plans and provide ongoing education and support to patients and their families. Patient care will be delivered within organizational philosophy, policy, and standards of community health practice. You will also be required to perform admission processes and case management responsibilities. This will be in accordance with relevant policies and standards of practice. Thorough and timely electronic documentation and interdisciplinary communication will be utilized.
