Ready for a rewarding assignment where you can make a meaningful difference in patients’ lives from the comfort of their homes? We are seeking an experienced Home Health RN for a 13-week contract in beautiful St. Albans, Vermont.
✨ Assignment Highlights
• 13-week contract with an ASAP start
• Tuesday–Saturday schedule, 8:00 AM–4:00 PM
• On-call rotation with hospice coverage required
• Holiday coverage required
• McKesson EMR
• Vermont RN license required
• Minimum 3 years of Home Health RN experience required
What You’ll Do
• Manage a home health caseload and serve as a patient care case manager
• Complete OASIS documentation accurately and on time
• Conduct patient and caregiver assessments and create individualized plans of care
• Provide skilled nursing services, including IV therapy, wound VAC care, complex wound care, medication management, and infection prevention
• Collaborate with physicians, LPNs, therapists, social workers, and the interdisciplinary care team
• Educate patients and families on treatments, medications, discharge planning, and available support services
• Coordinate referrals, transfers, discharges, and home care aide services
