Shifts: 8 Days; Monday-Friday 0800-1630 (rotating weekends and holidays); One weekend on call per month, one evening on call per week

Key Responsibilities of the RN - Home Health - Travel- Provide care for home health and hospice patients, including chronic condition management
- Conduct admissions, start of care (SOC), re-certifications, and resumptions
- Deliver wound care and practice excellent infection control in the home environment
- Proficiently document using OASIS for utilization review
- Participate in rotating weekend and evening on-call schedules
Requirements of the RN - Home Health - Travel- BLS Certification (AHA)
- Current RN license
- Recent home health experience within the last 2 years
- Proficiency in OASIS documentation
- Experience in wound care
