Home Health Registered Nurse (RN)

We are seeking a dedicated Home Health Registered Nurse (RN) to join our team. The ideal candidate will possess a strong background in certified home care and case management, specifically within a certified home health agency.

Job Summary

As a Home Health RN, you will be responsible for:

• Assessing patient conditions and needs.
• Implementing and coordinating care plans to meet patients' healthcare requirements.
• Providing ongoing education and support to patients and their families.
• Performing admission processes and case management duties in adherence to established policies and standards.
• Utilizing thorough and timely electronic documentation and interdisciplinary communication to enhance patient care.

Qualifications

• Minimum of 1 year of certified home care experience is required.
• At least 1 year of case management experience is necessary.
• Experience in hospice, private care, or as a standalone Case Manager does not fulfill the qualifications for certified home health care management experience.
• Must have the ability to travel to assigned cases as needed.
• Weekend rotations are required approximately once every 6 weeks.

Additional Information

The start date will be determined based on credentialing clearance. If not cleared by the designated start date, rescheduling of the start date will be necessary.
