Job Description – Registered Nurse (Home Health) | Smyrna Health & Wellness Center, Smyrna, DE Assignment Details- Facility: Smyrna Health & Wellness Center – Smyrna, Delaware
- Position: Registered Nurse – Home Health
- Duration: 13 Weeks
- Start Date: ASAP (Scheduled Orientation)
- Schedule: Monday–Friday, 8-hour day shifts
- Guaranteed Weekly Hours: 40
- Pay Cycle: Weekly Pay
- Coverage Area: Lower New Castle County, Kent County, and up to a 60-mile radius from Smyrna (including Southern Delaware).
- Required Experience: Recent Home Health RN experience with OASIS proficiency.
- License Required: Active Delaware RN License.
On-Call & Weekend Requirements- Participate in on-call rotation approximately 2 times per month (maximum of 3).
- Initial after-hours calls are screened by a non-clinical call center, with RNs receiving only true clinical concerns.
- On-call hours are generally 4:30 PM – 8:00 AM (occasionally split into evening and overnight shifts).
- No patient home visits are conducted between 8:30 PM and 8:00 AM; overnight responsibilities are limited to telephone triage.
- Most overnight on-call shifts involve 1–2 patient calls during the early evening, with very few calls overnight.
- Participate in one weekend rotation every four weeks, typically Saturday and Sunday, with the option of working two Saturdays every four weeks based on scheduling needs.
- An alternate weekday off is generally provided to ensure nurses do not work more than five consecutive days.
Credentialing Requirements- Allow approximately 5 weeks for credentialing.
- Non-Delaware residents must travel to Delaware after acceptance for in-person fingerprinting.
- Fingerprint processing typically requires 3–4 weeks before credentialing is complete.
- Provide skilled nursing care to adult and geriatric patients in a home health setting throughout Lower New Castle County, Kent County, and surrounding areas within a 60-mile radius of Smyrna, DE.
- Perform comprehensive patient assessments, develop individualized plans of care, and implement physician-ordered treatments.
- Complete OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and Follow-up assessments accurately and in compliance with Medicare guidelines.
- Administer medications, wound care, IV therapy, injections, catheter care, ostomy care, and other skilled nursing interventions in the patient's home.
- Monitor patient conditions, identify changes in health status, and communicate significant findings promptly to physicians and the interdisciplinary care team.
- Educate patients and caregivers on disease management, medication administration, chronic illness management, home safety, infection prevention, and self-care techniques.
- Coordinate patient care with physicians, physical therapists, occupational therapists, speech therapists, social workers, and case managers to achieve optimal outcomes.
- Develop, update, and evaluate individualized care plans based on patient progress and clinical needs.
- Maintain accurate, timely, and compliant clinical documentation while meeting productivity expectations within an 8-hour workday.
- Ensure compliance with Medicare Conditions of Participation, agency policies, HIPAA regulations, and infection prevention standards.
- Travel independently between patient homes while managing an assigned caseload efficiently within the designated service area.
- Participate in interdisciplinary case conferences, quality improvement initiatives, and discharge planning activities.
- Deliver compassionate, patient-centered care while promoting independence, reducing hospital readmissions, and improving overall quality of life.
