Home Health Registered Nurse (RN)

Job Summary

The Home Health Registered Nurse (RN) is responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within the organization's philosophy, policies, and standards of community health practice. This role also includes performing admission processes and managing cases in accordance with established policies and standards of practice, utilizing thorough and timely electronic documentation as well as effective interdisciplinary communications.

Key Responsibilities

• Assess patient conditions and develop appropriate care plans.
• Deliver patient care in alignment with community health standards and organizational policies.
• Provide ongoing education and support to patients and their families.
• Conduct admission processes for new patients.
• Manage care cases in accordance with regulatory standards.
• Utilize electronic documentation for timely and effective communication across interdisciplinary teams.

Qualifications

• Minimum of 1 year of experience in a Certified Home Care setting.
• At least 1 year of case management experience.
• Experience in hospice or private care alone does not qualify as home health care management experience.
• Ability to travel to assigned cases as needed.
• Weekend rotation is required every 6 weeks.
