Position Summary:
We are seeking an experienced RN – Home Health with a minimum of 2 years of recent home health experience to join our team. The ideal candidate will provide skilled nursing care to patients in their homes, ensuring high-quality, patient-centered care and effective care coordination. Candidates must hold active Oregon and Washington RN licenses and maintain current BLS certification.

 

NOTE: Locals Accepted: Yes - Considered "local" within 50-75 mile radius.

Roles & Responsibilities:

IMP:  To be eligible for this role, you must have both Oregon and Washington RN licenses.

  • Provide skilled nursing care to patients in their homes, including assessments, treatment, and ongoing monitoring of health conditions.
  • Develop, implement, and evaluate individualized care plans in collaboration with physicians and interdisciplinary team members.
  • Administer medications, wound care, IV therapy, and other prescribed treatments while ensuring patient safety and comfort.
  • Educate patients and families on disease management, medications, and home care procedures to promote independence and wellness.
  • Maintain accurate clinical documentation, coordinate services, and ensure compliance with home health regulations and agency standards.
