Roles & Responsibilities
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Educate patients and families on disease management and self-care
- Coordinate with interdisciplinary team for holistic patient care
- Document patient progress and communicate changes to physicians
Qualifications
- Active RN license in California
- Minimum 2 years of home health experience
- Current BLS certification
- Valid driver's license and auto insurance
- Experience with HomeCare HomeBase (HCHB) preferred
- Ability to work independently and manage a caseload
Knowledge & Skills
- Expertise in home health nursing and patient education
- Knowledge of CHF, COPD, wound care, and IV management
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Comfortable with EMR systems, especially Cerner
