The CHN intervenes to promote, maintain, or restore health and prevent illness. The CHN collaborates with healthcare providers, professionals, and community representatives to coordinate at-home care. The CHN evaluates the individual's and family's response to nursing interventions. The role requires independent action and judgment, with adherence to guidelines and protocols specific to the unit. The CHN utilizes time management, communication, planning, delegating, and problem-solving skills in providing care. Critical thinking skills are essential. The CHN must maintain emotional stability in emergency situations. The CHN is expected to exercise independent judgment and demonstrate strong interpersonal skills. Supervisory abilities are required, along with maintaining confidentiality. The CHN must effectively develop and maintain records and communicate clinical data both verbally and in writing. Flexibility regarding work hours is needed. The CHN should be able to drive in inclement weather and be willing to accept on-call responsibilities. Dependability in meeting the demands of a committed work schedule is essential. Requested time off during the assignment must be clearly noted. No more than five days of requested time off will be accepted. The CHN must have a working knowledge of home care reimbursement and regulatory guidelines. A history of flexibility in assuming varied assignments is preferred.
