Job Title: Community Health Nurse Profession: Registered Nurse (RN) Specialty: Community Health Duration: 13 weeks Shift: 8:00 AM - 4:30 PM Hours per Shift: 8 hours Experience: One year of acute nursing care, home health experience required License: Massachusetts RN license required Certifications: Current American Heart Association BLS certification required Must-Have: - Agency Massachusetts Temporary Nursing Service Agency License required - Certified Home Health required, including proficiency with OASIS - Demonstrated knowledge of community health principles and practices - Ability to function independently with minimal supervision or assistance - Strong critical thinking and decision-making skills - History of dependability in meeting work commitments Description: The Community Health Nurse provides, teaches, supervises, and documents nursing care for individuals at home. The nurse intervenes to promote, maintain, or restore health, prevent illness, and effect rehabilitation. This role requires collaboration with other healthcare providers and community representatives to coordinate home care. The nurse evaluates individuals and families' responses to interventions. Independent action and judgment are essential, along with the ability to manage time effectively. Strong communication, planning, delegating, and problem-solving skills are necessary for providing care. The role involves maintaining emotional stability in emergency situations. Candidates must demonstrate the ability to supervise others while maintaining confidential information. Developing and maintaining records and communicating clinical data in verbal and written form is required. Flexibility regarding work hours is necessary, along with the ability to drive in inclement weather. On-call responsibilities may be part of this role, and candidates must conform to customer service and performance standards.
