Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy, and standards of community health practice. Required to perform admission processes and case management responsibilities in accordance with policies and standards of practice.
