Shift: Days | 8-Hour Shifts (8:00 AM – 4:30 PM)
Guaranteed Hours: 40 Hours Per Week

We are currently seeking an experienced Registered Nurse (RN) – Community Health (Home Health) for a travel assignment in Pittsfield, Massachusetts. Requirements:

• Active Massachusetts Registered Nurse (RN) License
• Minimum 1 year of acute care nursing experience (hospital experience preferred)
• Home Health experience required
• Certified Home Health experience with OASIS proficiency required
• Current American Heart Association (AHA) BLS Certification
• Valid Driver's License, auto insurance, and reliable personal vehicle
• Strong assessment, communication, critical thinking, and time management skills
• Ability to work independently with minimal supervision
• Knowledge of community health principles and home care regulations
• BSN preferred
• Infusion Therapy Certification preferred
• Previous travel nursing experience preferred

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Assess, plan, implement, and evaluate individualized patient care plans.
• Perform comprehensive patient assessments and document findings accurately.
• Complete OASIS documentation and maintain accurate clinical records.
• Administer medications, wound care, IV therapy, and other physician-ordered treatments.
• Educate patients and families on disease management, medications, and home care.
• Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
• Monitor patient progress and adjust care plans as needed.
• Maintain compliance with agency policies, regulatory guidelines, and quality standards.
• Manage daily caseload efficiently while providing compassionate, patient-centered care.
