Home Health Registered Nurse (RN) – Travel Contract

Location: Red Bluff, CA 96080-4327
Pay: $3,000/week + Mileage (per GSA guidelines)
Shift: 8-hour Days
Schedule: 5 days/week | Every Other Weekend (EOW)

Position Overview

We are seeking a compassionate Home Health RN to provide skilled nursing care to patients in their homes. This role focuses on managing high-risk, recently discharged patients to prevent rehospitalization and promote recovery.

Required Qualifications
  • Active RN License (CA or Compact) – Required
  • BLS Certification – Required
  • Valid Driver’s License & Auto Insurance – Required
  • Experience with HomeCare HomeBase (HCHB) – Preferred
Patient Population
  • Adult & Geriatric patients (No Pediatrics)
  • Common conditions include:
    • CHF (Congestive Heart Failure)
    • COPD
    • Wound care / wound vacs
    • IV therapy (PICC lines)
Key Responsibilities
  • Conduct home visits and provide skilled nursing care
  • Perform Start of Care (SOC), Recertifications, and Routine Visits
  • Educate patients on disease management and equipment use
  • Coordinate care to reduce hospital readmissions
  • Document visits in EMR accurately
Productivity Expectations
  • 5.25 points/day average
    • Start of Care: 2.5 points
    • Recertification: 1.5 points
    • Resumption of Care: 1.75 points
    • Routine Visit: 1 point
  • Typically ~5 visits/day (varies with SOC volume)
Scheduling & Work Environment
  • Schedule finalized daily (subject to changes based on patient needs)
  • Preferred schedule: Tuesday–Saturday (flexible option for Monday–Friday)
  • Weekend Requirement: Every other weekend
  • Holiday Requirement: Yes (Thanksgiving and/or Christmas preferred coverage)
  • No call or standby requirement
  • Coverage: Home Health only (no floating)
Support & Team
  • Manager: Holly Haynes (available during working hours; on-call support after hours)
  • Administrator on-call for urgent needs
  • Home Health Aide support available
Additional Details
  • Equipment: Patient education for CPAP/BiPAP (no advanced RT responsibilities)
  • Dress Code: Any work-appropriate scrubs
