Key Responsibilities of the RN - Home Health - Travel- Perform comprehensive nursing assessments upon patient admission, considering all health factors.
- Develop data-driven care plans based on identified patient needs and case management objectives.
- Utilize HHRG values to guide visit frequencies and interdisciplinary resources appropriately.
- Refer patients to community resources as required.
- Participate in inter-disciplinary patient case conferences and evaluate care plan effectiveness.
- Provide supervision and documentation for Home Health Aide care plans bi-weekly.
- Communicate significant changes in patient conditions to relevant care providers.
- Complete all necessary paperwork and narrative documentation accurately and on time.
- Obtain and verify medical orders for all delivered care.
- Plan discharges appropriately and document all discharge procedures.
Requirements of the RN - Home Health - Travel- Successful passage of VNA pre-hire medication test
- Current RN license in Massachusetts
- Minimum one (1) year of nursing experience, or six months with a BSN
- CPR certification
- Preferred: Home care and IV experience, BSN, computer skills
