Job Responsibilities

• Utilize thorough and timely electronic documentation for patient records.
• Engage in interdisciplinary communication to ensure comprehensive patient care.

Qualifications

• Minimum of 1 year of Certified Home Care experience.
• Minimum of 1 year of Case Management experience.
• Experience must be specifically within a certified home health agency; other types of care or case management experience are not qualifying.
• Ability to travel to assigned cases.
• Weekend rotations required every six weeks.
