Job Description

Mass General Brigham Home Care is seeking an experienced Home Health RN for a 13-week assignment covering the Beantown/North Region.

The RN will provide skilled home health care, assess patient conditions, develop and implement care plans, complete admissions, provide patient/family education, and manage cases in accordance with MGB Home Care standards.

 

This role requires the ability to travel between assigned patient cases.

Required Experience

  • 1+ year of Certified Home Health Care experience required
  • 1+ year of Case Management experience required
  • Experience must be specifically within a certified home health agency

Service Area

Cases may be assigned throughout:

Back Bay • Beacon Hill • Boston • Cambridge • Charlestown • Chinatown • Fenway • Financial District • Kenmore • North End • Prudential • South Boston • South End • West End

Assignment Highlights

  • Day shift: 8:00 AM – 4:30 PM
  • No on-call requirement
  • Weekend rotation only once every 6 weeks
  • 13-week assignment
  • Home health + case management
  • Boston-area service territory
  • Start date is dependent on credentialing clearance
