Roles & Responsibilities
- Manage a caseload of roughly 30 adult patients in their homes
- Perform Start of Care assessments, recertifications, and daily visits
- Coordinate patient care plans and collaborate with interdisciplinary teams
- Document patient progress using Homecare Homebase (HCHB) system
- Educate patients and families on health management and home safety
- Provide skilled nursing care following physician orders and clinical guidelines
Qualifications
- Active RN license in Kentucky (or compact) required
- Minimum 1 year of Med-Surg experience in a hospital setting
- At least 1 year of Home Health experience mandatory
- Proficiency with Homecare Homebase (HCHB) system required
- Current BLS certification from AHA required
- Valid driver's license and reliable transportation (no mileage reimbursement)
Knowledge & Skills
- Strong clinical assessment and critical thinking skills
- Excellent time management and organizational abilities
- Comfort with independent decision-making in the field
- Proficient in electronic health records, specifically HCHB
- Exceptional communication and patient education skills
