Seeking an experienced Home Health RN Case Manager.
Home Health and HCHB experience required.
Territory covers the West Side of Portland, with travel up to a 60-mile radius.
EMR: HCHB.
Productivity expectation: 25 points/week.
Orientation: Day 1 in office.
Weekends: Not required.
On-call: Not required; candidates open to occasional emergency staffing may be contacted.
Company-issued device: Tablet or phone.
Must be comfortable working independently in a field-based home health environment.
