RN Case Manager – Home Health
StartDate: 8/24/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager – Home Health

POSITION DUTIES – Responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician’s order.  In addition, this position serves as Case Manager in cases in which this position is the lead discipline.

MINIMUM REQUIRED QUALIFICATIONS –

• VT/compact RN license
• BLS
• 1+ year home health experience
• Case management experience
• Oasis experience

PREFERRED QUALIFICATIONS –

• Experience as a home care traveler with proficiency in OASIS
• Homecare Home Base experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Days, Monday-Friday (with weekend rotation approximately every 4th week)

SYSTEMS – Oasis, Homecare Home Base

START DATE – ASAP
