Job Summary: Home Health Hospice Nurse in West Virginia. This position involves providing hospice care to terminally ill patients in their homes or nursing facilities within a designated seven-county area. The role requires working with two registered nurses to deliver compassionate care through patient visits, ensuring adherence to clinical standards and patient comfort.
Job Details:
Schedule: Dayshift, 8 hours per day, minimum of 40 hours per week
Location: Elizabeth, West Virginia (covering Harrison, Barbour, Upshur, Lewis, Taylor, Doddridge, Wetzel counties)
Requirements: BLS (American Heart Association); West Virginia nursing license; minimum of 2 years experience in hospice or related home health care settings
Certifications: BLS (AHA)
EMR: EPIC system training required, with orientation provided
Patients: Terminally ill individuals with conditions such as cancer, heart disease, lung disease, dementia, and other end-stage diseases
Shift Notes: 8-hour days at the Clarksburg location, working across multiple counties, with care focused on terminally ill patients in their homes or nursing facilities
Coverage Area: Above counties, with specific orientation on EPIC EMR and collaboration with another PT
Holidays & Weekends: Not required
Uniform: Royal Blue scrubs
Note: Must have hospice experience of 1 year or greater
