Key Responsibilities of the RN - Home Health:
Provide skilled nursing care to patients in their homes according to individualized care plans
Conduct comprehensive patient assessments and monitor health status changes
Administer medications, treatments, wound care, and other prescribed therapies
Educate patients and family members on disease management, medications, and healthy living practices
Coordinate care with physicians, therapists, case managers, and other healthcare professionals
Maintain accurate and timely clinical documentation in accordance with agency and regulatory requirements
Ensure patient safety and promote independence in the home environment
Respond effectively to patient concerns and changes in condition
Requirements:
Minimum 1 year of RN experience, preferably in Home Health, Med-Surg, Rehabilitation, or a related setting
Active Nebraska RN license
Current BLS certification required
Strong assessment, communication, and organizational skills
Ability to work independently and manage multiple patient visits effectively
Reliable transportation and valid driver's license may be required
