About the Position
Job Summary:
We are seeking an experienced Home Health Registered Nurse (RN) with strong OASIS documentation experience to provide skilled nursing care, case management, and coordination of services for patients in the home setting. This role is responsible for comprehensive patient assessments, development and implementation of individualized care plans, patient/caregiver education, interdisciplinary collaboration, and timely regulatory documentation. The ideal candidate will be highly organized, clinically strong, and comfortable managing an independent caseload.
Key Responsibilities:
- Utilize the nursing process to assess patient and caregiver needs and provide quality skilled nursing care in the home setting.
- Develop, implement, and communicate individualized plans of care in collaboration with patients, caregivers, and providers.
- Serve as Case Manager for assigned patients, managing caseloads and visit schedules according to productivity standards.
- Complete accurate and timely OASIS documentation in accordance with agency, state, and federal requirements.
- Stay current with changes to OASIS guidelines and documentation standards.
- Perform skilled nursing interventions including medication management, wound care, IV therapy, catheter care, injections, and disease management.
- Maintain competency with high-tech nursing skills such as IV use/maintenance, wound VAC, and complex wound care.
- Monitor patient response to treatment and revise care plans as needed.
- Collaborate with LPNs, Home Care Aides, therapists, social workers, and other team members to ensure continuity of care.
- Delegate tasks appropriately within scope of practice and provide ongoing supervision as needed.
- Educate patients and caregivers on medications, side effects, treatment options, reportable symptoms, discharge planning, and available services.
- Coordinate care with physicians, providers, schedulers, vendors, and interdisciplinary team members.
- Complete required forms and notices including Medicare and agency documentation accurately and on time.
- Participate in case conferences, IDT meetings, peer review audits, and quality improvement activities.
- Report incidents, infections, injuries, and safety concerns per policy.
- Coordinate safe patient discharges, transfers, and transitions of care.
- Advocate proactively for patient and family needs to ensure quality, individualized care.
- Maintain infection prevention and safety standards throughout the plan of care.
Requirements
Required Qualifications:
- Current Registered Nurse (RN) license in applicable state.
- Graduate of an accredited School of Nursing.
- OASIS experience required.
- Previous Home Health nursing experience required/preferred.
- Current BLS/CPR Certification required.
- Valid driver's license and reliable transportation required (if field-based role).
Preferred Qualifications:
- Experience with McKesson or similar home health documentation systems.
- Experience with hospice or interdisciplinary team-based community care preferred.
- Strong wound care, IV therapy, and chronic disease management experience preferred.
