Key Responsibilities of the RN - Home Health:
Deliver skilled nursing care to patients in their homes based on individualized care plans
Conduct comprehensive patient assessments and monitor ongoing health conditions
Administer medications, treatments, wound care, and other physician-ordered services
Educate patients and caregivers on disease management, medications, and recovery plans
Coordinate care with physicians, therapists, social workers, and other healthcare providers
Maintain accurate and timely documentation in accordance with agency and regulatory requirements
Identify changes in patient conditions and implement appropriate interventions
Promote patient safety, independence, and quality of life within the home setting
Requirements:
Minimum 1 year of RN experience, preferably in Home Health, Med-Surg, Rehabilitation, or a related specialty
Active Indiana RN license
Current BLS certification required
Strong clinical assessment and critical-thinking skills
Excellent communication and interpersonal abilities
Ability to work independently and manage multiple patient visits efficiently
Reliable transportation and valid driver's license may be required
