Responsibilities:- Provide direct patient care focused on comfort and symptom management.
- Develop and implement care plans with the hospice team.
- Educate patients and families on disease processes and end-of-life care.
- Coordinate care with interdisciplinary team members and community resources.
- Administer medications and monitor for side effects.
- Maintain accurate patient records.
- Offer emotional support to patients and families.
- Be available for on-call care when needed.
Qualifications:- OR Registered Nurse license
- BLS/CPR Certification
- Minimum 1-2 years' experience in hospice, palliative care, or nursing in a similar healthcare setting is required
- Valid driver's license
