Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Hospice Duration: 13 weeks Shift: 1 position - Monday to Friday, 1 position - Friday to Monday Hours per Shift: 8 hours Experience: Minimum of one (1) year as an RN, either in acute care hospital, oncology, long-term care, hospice, or home health. License: Current license to practice nursing in the state of requested employment. Certifications: BLS (American Heart Association) Must-Have: - Knowledge of terminally ill patients and their families. - Understanding of hospice principles and patient care needs. - Excellent patient assessment skills. - Good oral and written communication skills. Description: The Registered Nurse (RN) will provide direct patient care services to hospice patients. Supportive care will be provided to the patient's family. The RN will manage an interdisciplinary group of staff to enhance the quality of hospice care. Case management will be performed for all assigned patient clinical records. The RN will assure the highest quality of care is provided and documented in accordance with the Plan of Care. Perform initial and ongoing assessments of the patient. Assist in developing and implementing the Hospice Plan of Care prescribed by the physician. Work closely with other members of the Interdisciplinary Group to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services. Report any changes in the patient's condition to the attending physician and hospice team. Maintain accurate and updated medical records, including all interdisciplinary notes and assessments. Practice infection control measures and standard precautions. Monitor the scheduling of all staff. Participate in quality assessment and performance improvement programs. May be requested to participate in agency on-call schedules. Promote the company's core values consistently. This is a safety-sensitive position. Complete required compliance annual training. Perform other activities as assigned and attend staff meetings. Maintain a high level of knowledge pertaining to the management of pain and symptoms during the dying process. Perform routine supervisory visits of all team members at least monthly. Conduct ongoing review and follow-up on assigned patient clinical records to ensure quality services. Maintain primary control and professional management of each patient. Act as the primary liaison between the physician, patient/family, and hospice team. Be aware of all clinical issues, staffing, and scheduling concerns related to the patient. Educate patients, families, or caregivers on the needs identified in the Plan of Care. Provide supervision to Aides performing patient care. Prepare written instructions for patient care that are consistent with the interdisciplinary plan. Conduct supervisory visits to assess the performance of Aides' services. Ensure annual direct observation of Aides' performance to confirm competency. An RN must be available at all times to provide or supervise nursing care. This position may require travel within a certain radius.
