The Primary Hospice Registered Nurse serves as a vital member of the interdisciplinary team, focusing on symptom management, patient education, and supporting caregivers.

You will manage an assigned caseload, developing personalized goals of care and ensuring comprehensive support for patients navigating end-of-life journeys. Key Responsibilities

• Provide compassionate, holistic nursing care to hospice patients.
• Manage an assigned caseload of patients across diverse settings (private homes, nursing homes, group homes, assisted living facilities).
• Lead and collaborate effectively with the interdisciplinary care team.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Specialize in symptom management and pain control for hospice patients.

Required Qualifications

• Current Registered Nurse (RN) license.

Licensing Requirements

• Active and unrestricted Registered Nurse (RN) license in the State of Colorado.
