Job Description

Job Title: Registered Nurse Profession: RN Specialty: Hospice Duration: 13 weeks Shift: Weekend only Hours per Shift: 13/13/14 to make a 40-hour work week, dependent on patient appointment schedule Experience: Minimum of 2 years of professional nursing experience in the area of public health, such as an acute care or home care setting or hospice inpatient unit License: Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license as a Registered Nurse in a recognized NCSBN Compact State Certifications: Current CPR certification Must-Have: Valid driver’s license to make home visits Description: To provide nursing care for patients following established standards and practices in accordance with the Nurse Practice Act. To apply professional nursing theory and practice to assess, plan, implement, and evaluate patient care. Plans, organizes, and directs hospice care, supervising and coordinating care to patients with an array of terminal illnesses and compromised health states. Provides comprehensive skilled nursing services to qualified patients in their homes or hospice inpatient settings. Works with minimum daily supervision while performing interdependent practice in the field. Is responsible for supervising hospice aides, administering medications, symptom assessment and management, education/teaching, and end-of-life care. Communicates with patients, families, healthcare professionals, and community agencies to meet the care needs of the patient. Works in combination with the family/caregiver and the interdisciplinary group to meet the medical and emotional needs of the patient. Provides continuity of care through direct hands-on care that follows the patient’s plan of care. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition. Assesses patient condition, administers medications, and evaluates/treats patients’ symptoms and end-of-life needs. Completes, maintains, and submits accurate clinical notes/documentation regarding the patient’s condition and care provided. Attends interdisciplinary group meetings and actively participates in quality assessment and performance improvement teams. Assumes responsibility for personal growth and development and maintains professional knowledge and skills through continued education. Supervises care provided by the LPN and hospice aides to ensure treatment goals are being met and delegates responsibilities when required. Coordinates with other disciplines, agencies, families, etc., to ensure care needs of the patient are met. Participates in on-call duties as defined by the on-call policy. Performs other job duties and responsibilities as required. Demonstrates comprehensive physical assessment skills and the ability to implement the nursing process and principles of teaching and learning. Demonstrates good organizational and time management techniques, along with the ability to independently prioritize workload. Demonstrates excellent observation, problem-solving, and verbal and written communication skills. Frequent standing, walking, bending, crouching, stooping, and driving are required. Frequent lifting/moving patients and use of finger/hand dexterity are necessary. Exposure to blood, body fluids, and communicable diseases may occur. Requires work under stressful conditions, deadlines, or irregular hours. Requires occasional on-call evenings and weekends based on departmental needs.
