Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Hospice Care Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum of one (1) year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health. License: Current license to practice nursing in the state of employment Certifications: BLS (American Heart Association), current automobile insurance, and valid driver's license Must-Have: HCHB experience required Description: The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patient's family. The RN will manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care. The RN will case manage all assigned patient clinical records. The RN ensures that the highest quality of care is provided and documented in accordance with the Plan of Care. The RN will perform the initial and ongoing assessment of the patient. The RN helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with other members of the Interdisciplinary Group. The RN will work closely with all members of the Interdisciplinary Group to provide all-inclusive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support, to the patient and family. The RN will report any changes in the patient's condition to both the Attending Physician and the Hospice Physician. The RN maintains an accurate and updated medical record, including all interdisciplinary notes, Plans of Care, initial and ongoing assessments, and any other pertinent information. The RN practices infection control measures and standard precautions. The RN will monitor scheduling for all staff. The RN may participate in the Quality Assessment and Performance Improvement Program. The RN will complete required compliance training. The RN may perform other activities as assigned and attend staff meetings. The RN maintains a high level of knowledge regarding the management of pain and symptoms in the dying process. The RN conducts routine supervisory visits with all team members at least monthly. The RN performs ongoing reviews and follow-ups on assigned patient clinical records to assure quality services, standards of practice, and compliance with policies and regulatory requirements. The RN maintains close contact with the patient's family and/or significant others to provide information, support, and continuity of care. The RN is responsible for the education of the patient, family, or other caregivers relating to needs identified on the Plan of Care. This includes medication administration, dressing changes, oxygen administration, and understanding hospice philosophy and care of the terminally ill patient. The RN will supervise Aides and perform home visits at least every two weeks to assess care and services provided. The RN will prepare written assignments for Aides.
