Job Title: RN Care Manager Profession: Registered Nurse (RN) Specialty: Hospice Duration: 13 weeks Shift: Flexible M-F 8:30 AM - 5:00 PM, will consider 4/10's Hours per Shift: 8.5 Experience: 1+ year experience in hospice required License: Valid driver's license required Certifications: N/A Must-Have: Good driving record and reliable transportation Description: We are seeking an RN Care Manager to bring comfort, compassion, and quality care to patients and families when they need it most. Your ability to manage patient care and provide services requires substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education. Excellent written and verbal communication, problem-solving, and decision-making abilities are essential. The candidate should demonstrate experience in developing and maintaining rapport with patients and families. An ability to work well with an interdisciplinary team is required. The position primarily works in the patient’s residence. Travel to long-term care facilities, hospitals, and various agencies and businesses will be necessary. Travel within the service area will also be necessary. Overnight travel outside the service area may be necessary for continuing education. The employee is at direct risk for exposure to infections and bloodborne pathogens.
