Patients typically have a prognosis of six months or less. Common diagnoses include heart failure, heart disease, COPD, and various cancers. Other diagnoses may include Parkinson's disease and Alzheimer's disease. The nurse will manage a caseload of 14-16 patients or assist other case managers with routine visits. Visits are typically conducted 1-2 times per week and last around one hour each. Nurses must have reliable transportation to visit patients in their homes. Regular floating may be required between departments and facilities as needed. The position includes being on-call for one night shift every other week. There is flexibility in start and end times if needed. Patients often require support in pain management and end-of-life care. The role involves patient and family education and emotional support during the dying process. A strong awareness of cultural sensitivities is essential in delivering care.
