Job Title: Hospice Registered Nurse Profession: Nursing Specialty: Hospice Care Duration: 13 Weeks Shift: Monday to Friday Hours per Shift: 8 Hours Experience: Minimum of one year as a Registered Nurse in acute care hospital, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered in individual cases. License: Current license to practice nursing in the state of employment. Certifications: BLS (American Heart Association). Current automobile insurance and valid driver’s license. Must-Have: Knowledge of terminally ill patients and families. Understanding of hospice care and the special needs of hospice patients and families. Excellent patient assessment skills. Good oral and written communication skills. Thorough knowledge of nursing practice and managed care principles. Ability to operate nursing assessment tools and basic office equipment. Description: The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to the patients' families. The RN will manage an interdisciplinary group consisting of Licensed Practical Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care. The RN will case manage all assigned patient clinical records and ensure the highest quality of care is provided and documented in accordance with the Plan of Care. The RN will perform the initial and ongoing assessments of patients. The RN will help develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the Interdisciplinary Group. The RN will work closely with other members of the Interdisciplinary Group to provide all-inclusive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support, to the patient and family. The RN will report any change in the patient’s condition to the Attending Physician and Hospice Physician. The RN will maintain an accurate and updated medical record, including all Interdisciplinary notes, Plans of Care, initial and ongoing assessments, and any other information pertinent to the care of the patient. The RN will practice infection control measures and standard precautions. The RN will monitor the scheduling of all staff. The RN may be requested to participate in the agency on-call schedule. The RN will consistently promote core values and comply with safety standards. The RN will participate in the Quality Assessment and Performance Improvement Program. The RN will attend staff meetings. The RN will maintain a high level of knowledge pertaining to the management of pain and symptoms in the dying process. The RN will make routine supervisory visits with all other team members at least monthly. The RN will perform ongoing reviews and follow-ups on assigned patient clinical records to ensure quality services are provided. The RN will maintain close contact with the patient’s family and/or significant others to provide information, support, and continuity of care. The RN will act as the primary liaison between the physician, patient/family, and hospice team. The RN will be responsible for educating the patient, family, or other caregivers in relation to needs identified on the Plan of Care, including medication administration, dressing changes, and oxygen administration.
