We are seeking a compassionate and skilled Hospice Registered Nurse for a 13-week contract assignment based out of Sunnyvale, California. This role involves providing comprehensive, home-based hospice care to patients and their families across San Francisco, San Mateo, and Santa Clara Counties.
The Hospice RN is responsible for admitting new patients to service, conducting thorough physical, psychosocial, environmental, safety, and developmental assessments, and developing individualized plans of care. This position requires strong clinical judgment, meticulous documentation, and effective collaboration with a multidisciplinary team to ensure holistic patient and family support during end-of-life care.
Key Responsibilities
Admit patients to hospice services, initiating the plan of care.
Conduct comprehensive assessments of the patient's and family's physical, psychosocial, environmental, safety, and developmental needs.
Develop, implement, and evaluate individualized plans of care based on assessment findings.
Document observations, interventions, and patient responses accurately, including environmental factors.
Coordinate and collaborate effectively with a multidisciplinary healthcare team to ensure comprehensive patient care.
Participate actively in the hospice program's Quality Assessment Performance Improvement (QAPI) initiatives.
Perform follow-up visits as needed to monitor patient condition and adjust care plans.
Required Qualifications
Active Registered Nurse (RN) license.
Minimum of one (1) year of recent hospice nursing experience.
Ability to travel extensively within San Francisco, San Mateo, and Santa Clara Counties.
