Job Title: Home Health/Hospice RN
Facility Type/Setting: Home Health & Hospice – Outpatient Care
Shift & Schedule: Fri, Sat, Sun, and Mon; 8:00 AM–6:00 PM
Contract Length: 24 weeks
Guaranteed Hours: 40 hours/week

Call / Weekend / Holiday Requirements: Some call may be scheduled, but only on an emergency basis; Friday–Monday schedule includes weekends

Experience Required / Preferred: OASIS experience required; Home Health and Hospice experience required; experience traveling from patient to patient for home visits

Requirements:

• OASIS (Outcome and Assessment Information Set) experience
• Home Health and Hospice experience
• Experience conducting patient-to-patient home visits
• Ability to manage and adjust to schedule changes
• Valid driver's license in good standing
• BLS required

Responsibilities:

• Provide outpatient care through home visits
• Travel from patient to patient for home visits
• Manage and adjust to changes in the schedule
• Provide care in the Home Health and Hospice setting
