• Additional Information: The position requires providing hospice care services per facility protocols, completing patient assessments, coordinating with the interdisciplinary team, and maintaining detailed documentation. On-call shifts involve providing support for after-hours patient needs and collaborating with inpatient teams. These shifts include four consecutive days with on-call duties from 5 PM to 8 AM.
