Developing patient daily list, Review of Patient charts, assessing patient needs, Opening Goals of care discussions, Education on disease process, Advance Directives and POLSTs, Holds family meetings as needed, End of Life discussions/support. Attends daily Rounds. Experience or 'Must have' skillsMust have 2-5 years of recent experience in palliative and/or hospice setting, ICU and or oncology Experience a plus. Strong Acute Care, clinical thinking and communication skills necessary. Flexibility and strong organizational skills a must have. Desire to serve others - patient's families, team members a must. Charge Nurse/House Supervisor (Is the charge nurse required to take a patient assignment?)Palliative Care RN Coordinator: No hands on patient care.Shifts & Scheduling (Do you allow self-scheduling?
