TLC Nursing is seeking a travel nurse RN Hospice for a travel nursing job in Portland, Oregon.

Job Description & Requirements

**Title**  
Hospice RN – Home Health / Hospice

**Schedule**  
- Shift: 8-hour days only

**Assignment Details**  
- Guaranteed Hours: Facility may call off up to one shift per pay period

**Requirements**  
- Licensure: RN license in two states
- Experience: Minimum 1 year of skilled hospice experience required
- Certifications: BLS certification required
- Clinical Skills:  
 - Experience in hospice/home health care and palliative nursing 
 - OASIS documentation experience 
 - Pain assessment and management skills 
 - Patient/family education and support 
 - Knowledge of advanced directives and end-of-life care 
 - Basic IV skills 
 - Symptom management skills (e.g., nausea, constipation)

**Roles & Responsibilities**  
- Provide direct patient care and support to patients in a home health setting
- Conduct assessments and develop care plans based on individual patient needs
- Educate patients and families on hospice care and services
- Collaborate with interdisciplinary teams to ensure comprehensive patient care
- Facilitate symptom management and pain control measures
- Maintain accurate documentation and compliance with care standards
