Description:
Our home health and hospice agencies serve the entire community and offer personalized in-home care.
We are seeking an experienced RN Care Manager to provide comprehensive end-of-life care to terminally ill patients.
This role involves managing a caseload of patients while conducting thorough assessments and developing individualized care plans.
The RN will provide skilled nursing interventions to address physical, emotional, and psychosocial needs.
You will serve as the primary point of contact for patients, families, and the interdisciplinary team.
Effective communication and continuity of care are essential components of this role.
Responsibilities include handling symptom management, medication administration, and patient and family education.
You will coordinate with physicians and support staff to ensure ongoing evaluation of care effectiveness.
A key function is to conduct initial and ongoing assessments of patients' needs to update care plans accordingly.
Additionally, the RN will deliver skilled nursing care including medication management, pain and symptom control, and wound care.
Providing emotional support and crisis intervention to patients and families as needed is also part of the job.
You will manage a typical caseload of 12-18 patients while ensuring continuity of care.
Regular communication with team members is crucial for success in this position.
Participation in interdisciplinary team meetings to review and revise care plans is expected.
You must maintain timely and accurate documentation in compliance with regulatory requirements.
Ensuring compliance with all Medicare, Medicaid, and state/federal regulations is mandatory.
Participation in the on-call rotation and prompt response to after-hours patient needs are required.
Other job-related duties may be assigned as needed.
