Duties & Responsibilities:
Providing compassionate care, strength and courage to patients and families.
Helping patients identify their own personal goals of care and helping caregivers have confidence in caring for their family member.
Acting as a generalist on all diseases and a specialist in symptom management, requiring excellence in assessment and critical thinking.
Managing an assigned caseload of patients and leading the interdisciplinary team caring for those patients.
Providing care wherever patients call home – private homes, nursing homes, group homes, or assisted living facilities.
Maintaining a weekday schedule of Mon-Fri 8am-5pm with occasional back-up for weekends and on-call.
