Pain & Symptom Management: Assess patients and administer medications/treatments to ensure comfort and alleviate suffering.Care Planning: Develop and update customized patient care plans in collaboration with physicians, social workers, and other team members.Family Support: Educate family caregivers on disease progression, what to expect, and medication management.Crisis Intervention: Respond quickly to sudden changes in a patient's condition to manage pain and provide crisis support.Documentation: Keep accurate, up-to-date patient medical records and facilitate communication between the care team.Emotional & Spiritual Care: Offer compassionate companionship, grief counseling, and post-mortem support to grieving relatives. • 2 years Hospice experience - [REQUIRED}

• BLS (AHA)
