Pinellas county - assignment based on zip codes within a team's territory No on call or weekend visits Caseload size 17-20 depending on if assigned to a home or facility team Minimum 20 patient visits each week HomeCare HomeBase Productivity points minimum 25 40 hours/week 8a-5p Required Skills & Experience: -Minimum of one (1) year of related experience and/or training. OR -Equivalent combination of education and experience. Preferred Skills & Experience: -Prefer one (1) year experience in an acute or post-acute care setting. -Preferred experience caring for terminal patients and others with life threatening illnesses. -Ability to type 25 wpm is highly preferred. BLS Certification Florida State Driver s License Registered Nursing License
