Job details:
Position: Registered Nurse – Hospice SNSNSN
Location: Plymouth, MA.
Shift: Days – 4x8s (7:30 AM – 4:00 PM)
Duration: 13 weeks
Job Duties:
• Provide hospice nursing care in outpatient settings, patient homes, and healthcare facilities.
• Conduct 4–5 patient visits per day for routine hospice care and case management.
• Travel approximately 30–50 miles daily between patient locations.
• Coordinate care plans and collaborate with interdisciplinary hospice teams.
• Document patient care and case management activities per facility guidelines.
