License: Active, unrestricted Registered Nurse (RN) license in the applicable state. Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. Nursing practices are based on evidence, current research, as well as policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities. This includes provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also part of the role. The RN provides patient and family-centered education. Care coordination and individualized treatment planning are conducted as applicable. The RN performs and documents comprehensive assessments in accordance with scope of practice and hospital policies. They consider the patient's culture, age, and developmental level. Identifying patient risk factors and presenting complaints is crucial. Objective and subjective health information is collected and verified. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Actions and interventions relevant to the patient's culture and age are designed. Desired outcomes are determined in coordination with the patient and family. The RN fully implements patient care therapies and procedures within their scope of practice. They explain procedures, treatments, and processes to patients and families. Initiating action to alleviate patient stress and discomfort is essential. Recognizing, responding to, and communicating changes in patient condition appropriately is required. Ongoing assessments and responses to therapies are conducted and documented. Tasks are delegated appropriately, overseeing team members in patient care activities. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients related to prioritized problems are identified and addressed. Facilitating the patient discharge process includes executing discharge orders. Input into patient/nurse assignments considers available staffing, unit activity, and patient acuity. The RN actively participates in unit-based activities in support of organizational values. A positive group dynamic and healing environment is promoted through actions. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Participation in department and/or unit quality improvement activities is encouraged. The RN uses a holistic perspective regarding patient care.
