Description:
The RN implements professional nursing practice centered on the patient as a whole person.

Responsibilities include coordinating and providing direct patient care activities through assessments and treatments.
Facilitates admission, transfer, and discharge activities.
Provides patient and family-centered education and individualized treatment planning as applicable.
Performs and documents comprehensive assessments according to nursing scope of practice.
Considers patient culture, age, and developmental level during assessments.
Identifies patient risk factors and collects health information from family or others as necessary.
Develops a problem list, prioritizing patient needs based on input from the healthcare team.
Designs actions and interventions relevant to the patient's culture and age.
Determines desired outcomes in coordination with the patient and family according to the medical plan of care.
Explains procedures, treatments, and processes to the patient and family.
Initiates action to alleviate patient stress and discomfort.
Recognizes, responds to, and communicates changes in patient condition appropriately.
Conducts ongoing assessments and documents responses to procedures and therapies.
Delegates tasks appropriately and oversees team members in patient care activities.
Monitors accuracy and completeness of delegated patient care provided by other team members.
Identifies and addresses educational needs of patients related to prioritized issues.
Facilitates the discharge process by executing orders in a timely manner.
Provides input into patient/nurse assignments considering staffing and patient acuity.
Actively participates in unit-based activities in support of organizational values.
Promotes a positive group dynamic and healing environment through actions.
Assures nursing excellence according to accepted standards and evidence-based practice.
Participates in department and unit quality improvement activities and core measures.
Uses a holistic perspective regarding patient care.
