The RN utilizes a theory of caring, relationship-based care, and a professional nursing practice model to assess, plan, implement, and evaluate care for all patients assigned to his/her care. The RN makes determinations regarding the progress of the patient toward goals and utilizes a therapeutic professional relationship with the patient, family, and interdisciplinary team to facilitate the promotion, maintenance, and restoration of health. Care is provided according to the specific physiologic, developmental, and psychosocial needs of the patient. The RN shall be considered competent when he/she consistently demonstrates the ability to transfer scientific knowledge from social, biological, and physical sciences in applying the nursing process. This includes providing care that is consistent with the interdisciplinary plan of care and grounded in the principles of relationship-based care. The RN assesses the patient's physical and psychosocial condition on an ongoing basis and formulates a nursing diagnosis through observation of the client's physical condition and behavior, and through interpretation of information obtained from the client and others. The care plan is formulated in collaboration with the client, ensuring that nursing care services provide for the client’s safety, comfort, hygiene, and protection, as well as disease prevention and restorative measures. The RN performs essential nursing skills, explains health treatments to the client and family, and teaches them how to care for the client’s health needs. The effectiveness of the care plan is evaluated through observation of the client’s condition, signs and symptoms of illness, reactions to treatment, and communication with the client and health team members, modifying the plan as needed. The RN demonstrates current working knowledge and safe use of equipment, instrumentation, and medications. Tasks are delegated to care team members based on their legal scopes of practice and capabilities, with effective supervision of the nursing care being given. The Clinical Nurse acts as the client’s advocate by initiating actions to improve healthcare or change decisions that go against the interests of the client, providing them the opportunity to make informed decisions about their healthcare. Culturally competent, age-appropriate, ethical care is provided within legal standards of practice. Coordination of patient care activities involves liaisons with nurses, physicians, allied health professionals, and ancillary care team members. Effective communication skills are utilized to facilitate safe patient hand-offs. Nursing care is documented in accordance with hospital policies. Manual and automated information systems are utilized to improve efficiency and productivity. Each individual contributes to a positive patient care experience through their interactions with patients and families. There is accountability to understand the impact of personal behaviors on patient experience. Assistance is provided in the resolution of complaints, aiming for immediate resolution if possible. Complaints from physicians, employees, and patients are investigated, providing information as necessary. Other duties may be assigned as needed. Proficient written, verbal, and electronic communication skills are required. All physical duties of the job must be performed, including lifting, bending, pushing, pulling, sitting, standing, walking, kneeling, twisting, reaching, and fine motor skills. Completion of annual organizational requirements for employment is required.
