Position Overview
We are seeking an experienced Labor & Delivery Registered Nurse (RN) to join a high-risk Family Birth Center providing comprehensive care across Labor & Delivery, Obstetrics, and Postpartum (LDRP). This role requires an RN who is confident managing patients throughout the entire birth process, including high-risk pregnancies, inductions, preterm labor, cesarean sections, and postpartum care.
The ideal candidate must be comfortable with couplet care, circulating C-sections, floating to Mother-Baby (MB), and managing patients from admission through delivery and postpartum recovery.

- L&D Beds: 10
- Postpartum Beds: 12
- OR Suites for C-Sections: 2
- Triage Beds: 3
- OB-ED / Antepartum Testing: 3 additional beds used for OB-ED overflow
- Birth Volume: Approximately 140–165 births per month
- Electronic Fetal Monitoring: GE Corometrics / Fetalink surveillance
- Patient Population: High-risk obstetric patients
Patient Ratios
- Labor & Delivery: 1:1–1:2
- Couplet Care: 1:3–1:4
- Antepartum: 1:1–1:3
- Ratios follow AWHONN guidelines
Common Patient Conditions
RNs may care for patients experiencing:
- Preterm labor
- Vaginal bleeding
- Labor inductions
- Cesarean sections
- Hypertension and pregnancy-related complications
- High-risk pregnancies
- Antepartum and postpartum conditions
Key Responsibilities
- Provide comprehensive nursing care throughout Labor, Delivery, Recovery, and Postpartum
- Manage high-risk obstetric patients in a fast-paced Family Birth Center
- Perform continuous maternal and fetal assessments
- Interpret fetal monitoring and recognize changes requiring intervention
- Circulate C-sections independently — required for all RNs
- Provide couplet care and postpartum nursing care
- Float to Mother-Baby when required
- Administer and titrate Pitocin
- Participate in emergency obstetric interventions and deliveries
- Provide care in accordance with AWHONN standards and facility protocols
- Collaborate closely with OB providers, anesthesia, neonatology, NICU, and the interdisciplinary care team

Required Experience & Skills
- Previous Labor & Delivery experience required
- Strong experience with LDRP/Family Birth Center environments
- Must be comfortable caring for high-risk OB patients
- C-section circulating experience required
- Must be able to float to Mother-Baby/Postpartum
- Must be comfortable providing couplet care
- Ability to manage and titrate Pitocin
- Strong fetal and maternal assessment skills
- Ability to read and interpret EKG strips on mothers in PACU
Staffing & Clinical Support
- Charge RN/House Supervisor: Generally does not take a patient assignment but provides break relief
- OB Surgical Technicians: Available on the unit
- OB Hospitalist: In-house 24/7
- Neonatology: On call 24/7 and rounds daily
- Anesthesia: In-house 24/7
- Respiratory Equipment: No ventilators on the unit
- Telemetry: RNs must interpret maternal EKG strips in PACU
Schedule
- 12-hour shifts
- Shift begins at 6:45 AM/PM
- Bedside report
- Every other weekend required
- Holiday rotation with staff
- Additional shifts and overtime may be available
- If placed on call, 30-minute response time is required
Floating Requirements
- No routine in-house floating
- Travelers may be required to float to other Sacramento-area facilities
- Mid-shift floating to another facility is rare but may occur
Scrubs
- Scrubs provided by the facility
Additional Role Information
The “Baby Catcher”/NRP role may be assigned for the entire shift. This responsibility may be covered by NICU or L&D staff depending on staffing availability.
Submission & Compliance Requirements
- 2 supervisory references required at submission
- 1 reference within the last 12 months
- 1 reference within the last 3 years
- Charge RN references are accepted
- Copy of driver's license required at submission to verify legal name
- Travelers will be scheduled to work 2 of the 3 major holidays: Thanksgiving, Christmas, and New Year's
- Holiday RTO may be considered but is not guaranteed
- Candidates must not have been directly employed (FT, PT, or PRN) by a facility affiliated with CommonSpirit, CHI, or Dignity within the past 12 months
- Candidates previously employed PRN through an outside staffing firm may be considered
- Position may be prescreened/auto-offered, with final approval subject to manager review; manager may rescind an offer within 24 hours
