Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. The RN honors patients' minds, bodies, and spirits by respecting human dignity and the uniqueness of everyone. Nursing practices are based on evidence and current research as well as hospital and department policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The Registered Nurse spends the majority of their time coordinating, ensuring, and providing direct patient care activities on the unit through provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities while providing patient and family-centered education is a key responsibility. Care coordination and individualized treatment planning are also essential components of this role. The RN performs and documents a comprehensive assessment in accordance with scope of practice and hospital policies and procedures. Consideration is given to the patient’s culture, age, and developmental level. Coordination of the initiation of the patient database is a priority. Identifying patient risk factors and presenting complaints is essential. The RN obtains patient health information from family or others as necessary. Objective and subjective data are collected diligently. Verification of initial admission assessment, if obtained by other members of the healthcare team, is crucial. A problem list is developed, prioritizing patient needs based on input from the inter-professional team as appropriate. Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care. Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy within the scope of practice. Explaining procedures, treatments, and processes to patients and families is an important duty. The RN initiates actions to alleviate patient stress and discomfort. Recognizing, responding to, and communicating changes in patient condition appropriately is essential. Ongoing comprehensive patient assessments and documentation of responses to procedures and therapies using the nursing process are conducted according to hospital policy and established standards. The plan of care and clinical pathway are revised and coordinated based on patient and family responses and outcomes. Mentoring of other staff members and nursing students is encouraged. Facilitation of new staff orientation is a responsibility. The RN acts as a role model and resource to others in their clinical role. Demonstrating support of professional governance and staff empowerment activities is expected. Active participation in unit-based activities in support of organizational values and priorities is essential. Promoting a positive group dynamic and healing environment through words and actions is critical. Assuring nursing excellence in accordance with accepted standards, evidence-based practice, current research, and a caring philosophy is a priority. Participation in quality improvement activities and monitoring of documentation is vital. Using a holistic perspective regarding patient care is essential. Demonstrating critical thinking and creativity in handling patient issues is required. Attendance at required safety training programs and the ability to describe responsibilities related to safety are expected.
